Keep up the pressure. The pressure to be transparent, to answer the tough questions and to understand what our Congress is voting on. We need our Congress to recognize that costs will increase not decrease. That there will still be millions of uninsured even after we spend an additional $1 trillion. That the only way costs will be controlled is rationing of health care resources. We need to understand that under the current proposals the economic rationing that we have today will be traded off for end of life rationing tomorrow. We need to understand that the US will lose its advantage with regard to developing health care breakthroughs and that no other country will come to take our place.

If IMAC can only recommend policy changes that do not increase Medicare outlays then it means that health care providers will essentially never see fee increases as time goes on. As the population ages, and as inflation marches merrily on, that is a recipe for disaster because Medicare fees are already so low that often health care providers are actually losing money providing various services to Medicare patients, which is why providers are dropping out of Medicare. The IMAC would insure that this situation continues to get worse, and that the proportion of foreign trained physicians in the United States continues to escalate as American physicians retire and are not replaced by American citizens. Trying to lower Medicare costs by reducing fees to those caring for these Medicare patients will not work since we're already being paid about 25% of what fee for service insurance plans pay.<div class="comment-remix-meta"><a href="#" class="replyto" onclick="replyto('4526','eyedoc'); return false;">Reply</a> - <a href="#" class="quote" onclick="quote('4526','eyedoc','If IMAC can only recommend policy changes that do not increase Medicare outlays then it means that health care providers will essentially never see fee increases as time goes on.

[...] CBO Kills the President’s Medicare Commission Proposal – Keith Hennessey – Hennessey is missing the point. He is right to quote CBO language that criticizes IMAC, but the CBO is not saying IMAC is a bad idea. They are saying it should be even stronger and more powerful, which, if you understand the history and economics of health care, makes perfect sense. (I should also note, for example, that if IMAC only has power over Medicare and not private insurers, then we can expect hospitals to shift resources to milk private insurers as they did after the last major Medicare reimbursement change under Ronald Reagan.)
